    Representativeness of samples from general practice lists in epidemiological studies: case-control study

    Ethical constraints often prevent epidemiological studies from evaluating the impact of non-participation. Particular problems may arise when subjects fail to respond to an approach by researchers or when they cannot be contacted because of inaccurate contact details or a doctor's refusal to give permission for their patient to be approached. If these subjects differ from those subjects who agree or decline to participate then the validity and generalisability of the study may be compromised. We investigated these issues in a case-control study of acute leukaemia in England

    Vitamin K policies and midwifery practice: questionnaire survey

    Objectives: To investigate policies on neonatal vitamin K and their implementation. Design: Two phase postal survey. Setting: United Kingdom. Participants: A 10% random sample of midwives registered with the United Kingdom Central Council for nursing, midwifery, and health visiting. Of 3191 midwives in the sample, 2515 (79%) responded to phase one and 2294 (72%) completed questionnaires on their current jobs (November 1998 to May 1999). In phase two, 853 (62%) of 1383 eligible midwives gave details on 2179 of their earliest jobs (start dates before 1990). Results: All the midwives in clinical practice at the time of the survey (2271, 99%) reported that they were working in areas with official policies on neonatal vitamin K. Seven distinct policies were described: intramuscular vitamin K for all babies (1159, 51.0%); intramuscular vitamin K for babies at "high risk," oral for others (470, 20.7%); oral vitamin K for all babies (323, 14.2%); parental choice for all (124, 5.5%); parental choice for all except babies at high risk, (119, 5.2%); intramuscular vitamin K for babies at high risk only (33, 1.5%); oral vitamin K for babies at high risk only (17, 0.7%); and a disparate group of policies including intravenous vitamin K for some babies (26, 1.1%). Previous policies were (and some may still be) open to individual interpretation and were not always followed. Conclusions: Hospital policy is not necessarily a good guide to individual practice. The primary purpose of clinical records is to document patient care, and recording practices reflect this. There is considerable variation in vitamin K policies and midwifery practice in the United Kingdom, and there is no clear consensus on which babies should receive vitamin K intramuscularly

    No Effect of Steady Rotation on Solid 4^4He in a Torsional Oscillator

    We have measured the response of a torsional oscillator containing polycrystalline hcp solid 4^{4}He to applied steady rotation in an attempt to verify the observations of several other groups that were initially interpreted as evidence for macroscopic quantum effects. The geometry of the cell was that of a simple annulus, with a fill line of relatively narrow diameter in the centre of the torsion rod. Varying the angular velocity of rotation up to 2\,rad\,s1^{-1} showed that there were no step-like features in the resonant frequency or dissipation of the oscillator and no history dependence, even though we achieved the sensitivity required to detect the various effects seen in earlier experiments on other rotating cryostats. All small changes during rotation were consistent with those occurring with an empty cell. We thus observed no effects on the samples of solid 4^4He attributable to steady rotation.Comment: 8 pages, 3 figures, accepted in J. Low Temp. Phy

    A Prototype System for Measuring Microwave Frequency Reflections from the Breast

    Microwave imaging of the breast is of interest for monitoring breast health, and approaches to active microwave imaging include tomography and radar-based methods. While the literature contains a growing body of work related to microwave breast imaging, there are only a few prototype systems that have been used to collect data from humans. In this paper, a prototype system for monostatic radar-based imaging that has been used in an initial study measuring reflections from volunteers is discussed. The performance of the system is explored by examining the mechanical positioning of sensor, as well as microwave measurement sensitivity. To gain insight into the measurement of reflected signals, simulations and measurements of a simple phantom are compared and discussed in relation to system sensitivity. Finally, a successful scan of a volunteer is described

    Childhood cancer and paternal employment in agriculture: the role of pesticides.

    Previous studies have suggested that the offspring of men potentially exposed to pesticides at work may be at increased risk of kidney cancer (Wilms' tumour), brain tumours, Ewing's bone sarcoma and acute leukaemia. This paper examines the association between potential occupational exposure of fathers to pesticides and offspring's death from cancer in a large national database. Records for 167703 childhood deaths occurring during 1959-63, 1970-78 and 1979-90 in England and Wales have been analysed. Among the offspring of men with potential occupational exposure to pesticides there were 5270 deaths, of which 449 were due to cancer. Associations were assessed using proportional mortality ratios (PMRs), with adjustment for age, year of death and paternal social class. Of the childhood cancers previously linked with potential paternal occupational exposure to pesticides, the only statistically significant excess was for kidney cancer (PMR=1.59, 95% CI=1.18-2.15, based on 42 deaths). Although these results offer some support for the suggestion that paternal occupational exposure to pesticides may be related to the subsequent development of kidney cancer in offspring, other explanations cannot be excluded. In the light of the findings presented here and elsewhere, further, more detailed, research into the nature of this relationship is warranted

    2012), The IMF dependence of the local time of transpolar arcs: Implications for formation mechanism

    [1] Transpolar arcs are auroral features that extend from the nightside auroral oval into the polar cap. It is well established that they occur predominantly when the interplanetary magnetic field (IMF) has a northward component (B z > 0). Results concerning how the magnetic local time at which transpolar arcs form might depend upon the IMF dawn-dusk component (B Y ) are more mixed. Some studies have found a correlation between these two variables, with Northern Hemisphere arcs forming predominantly premidnight when B Y > 0 and postmidnight when B Y < 0 and vice versa in the Southern Hemisphere. However, a more recent statistical study found that there was no significant correlation, and other studies find that the formation of moving arcs is triggered by a change in the sign of the IMF B Y component. In this paper, we investigate the relationship between the magnetic local time at which transpolar arcs form and the IMF B Y component. It is found that there is indeed a correlation between the magnetic local time at which transpolar arcs form and the IMF B Y component, which acts in opposite senses in the Northern and Southern hemispheres. However, this correlation is weak if the IMF is only averaged over the hour before the first emergence of the arc and becomes stronger if the IMF is averaged 3-4 h beforehand. This is consistent with a mechanism where the magnetic local time at which the arc first forms depends on the B Y component in the magnetotail adjacent to the plasma sheet, which is determined by the IMF B Y component during intervals of dayside reconnection in the hours preceding the first emergence of the arc. We do not find evidence for the triggering of arcs by an IMF B Y sign change

    Early Service leavers: a study of the factors associated with premature separation from the UK Armed Forces and the mental health of those that leave early.

    BACKGROUND: Approximately 18,000 personnel leave the UK Armed Forces annually. Those leaving before completing the minimum term of their contracts are called early Service leavers (ESLs). This study aims to identify characteristics associated with being an ESL, and compare the post-discharge mental health of ESLs and other Service leavers (non-ESLs). METHOD: A cross-sectional study used data on ex-Serving UK Armed Forces personnel. ESLs were personnel leaving before completing their 3-4.5 years minimum Service contracts and were compared with non-ESLs. Multivariable logistic regression was used to estimate odds ratios and 95% confidence intervals for the associations between Service leaving status with socio-demographics, military characteristics and mental health outcomes. RESULTS: Of 845 Service leavers, 80 (9.5%) were ESLs. Being an ESL was associated with younger age, female sex, not being in a relationship, lower rank, serving in the Army and with a trend of reporting higher levels of childhood adversity, but not with deployment to Iraq. ESLs were at an increased risk of probable post-traumatic stress disorder (PTSD), common mental disorders, fatigue and multiple physical symptoms, but not alcohol misuse. CONCLUSIONS: The study suggests that operational Service is not a factor causing personnel to become an ESL. Current mental health problems were more commonly reported among ESLs than other Service leavers. There may be a need to target interventions to ESLs on leaving Service to smooth their transition to civilian life and prevent the negative mental health outcomes experienced by ESLs further down the line

    The interaction between transpolar arcs and cusp spots

    Transpolar arcs and cusp spots are both auroral phenomena which occur when the interplanetary magnetic field is northward. Transpolar arcs are associated with magnetic reconnection in the magnetotail, which closes magnetic flux and results in a "wedge" of closed flux which remains trapped, embedded in the magnetotail lobe. The cusp spot is an indicator of lobe reconnection at the high-latitude magnetopause; in its simplest case, lobe reconnection redistributes open flux without resulting in any net change in the open flux content of the magnetosphere. We present observations of the two phenomena interacting--i.e., a transpolar arc intersecting a cusp spot during part of its lifetime. The significance of this observation is that lobe reconnection can have the effect of opening closed magnetotail flux. We argue that such events should not be rare
